Getting Started

Visit www.cda.gov.ph (please use Internet Explorer) and click the Online Issuance of New Certificate of Registration link. The link will take you to the Registration site. Click the Memorandum Circular (MC) link to download the MC.

The Log-on page
Enter your cooperative’s CIN and click OK. A cooperative is issued a 10-digit CIN by your concerned CDA Extension Office. Make sure that the correct CIN is entered. Your cooperative’s classification must be “operating” to be able to proceed to the next stage. To read through the procedures, scroll down the text on the frame; or click the download link at the bottom of the page.

The Application Form page

If you have entered the correct CIN and your status is “operating”, you will be directed to the Application Form page. Enter the following information as discussed below.

General Rules

Do not use ALL CAPS, except for your cooperative name’s acronym.

Do not use special characters like * / # @ ! or space to begin a name or telephone number

For unavailable information in text fields, input “NA”. For number fields, input “0”. The system will prompt you if you have entered an incorrect character.

All the information on the application form is derived from the CIN database, except for the BIR TIN and the year for total number of members and Total Assets. You may enter new or update your cooperative’s profile. However, for items that need supporting documents like the Amended Name or Amended Type, your cooperative should present the necessary documents to your concerned CDA Extension Office.

How to fill up the Application Form
Name of Cooperative
Enter your latest amended Name here. Capitalize only the first letters e.g. San Pedro Multipurpose Cooperative. Do not put characters e.g. _ * / . @ # or space before the Name. However, the exemption to this rule is when your cooperative name starts with an acronym e.g. TOREDECO.

Cooperative Identification Number (CIN)
The CIN field is disabled because it is your cooperative’s unique ID and should not be altered.

BIR TIN
Enter your 9-digit BIR TIN if available. Or enter zero if not yet available.

Total Assets

Enter your Total Assets as of 2007or 2008 or 2009. Type in the figures without commas, space or other characters e.g. 100000.35. Decimal point is accepted. The address section is already predetermined by your CIN. The system already loads the region, province, district and municipality which have been previously encoded by CDA. If the address loaded is not updated or incorrect, you have to update it by following the guide below.

Street Address
Enter ONLY the street address (number, name of street or barangay). If street number or name of street is not available, enter purok, sitio or barangay.

Region
Select the Region where your cooperative is located. Upon selecting the Region, you will notice that the page will flicker indicating that the Provinces which belong to the Region you selected are loaded on the Province dropdown field. By default, the region field is pre-determined by your CIN. For example, if your CIN was issued by Region 1, the default value of the Region field would be Region 1.

Province
Select the province where your cooperative is located. Once you have selected a province, the page will refresh automatically indicating that the appropriate Districts have been loaded to the District dropdown list. You will experience a flicker but this is normal. The speed of the flicker also depends on the speed of your computer and internet connection.

District
Select the district where your cooperative is located. Once you have selected a district, the page will refresh automatically indicating that the appropriate cities/municipalities have been loaded to the City/Municipality dropdown list. Again, you will experience a flicker but this is normal. The speed of the flicker also depends on the speed of your computer and internet connection.

City/Municipality
Select the city or municipality where your cooperative is located.

Sub-Category
Select a sub-category or cooperative type from the dropdown list. The dropdown list contains primary, secondary and tertiary cooperatives.

How to Submit your Application Form

After completing the Application Form, click the Submit button. A Print Requirements link will then appear below the page. Click this link to view the requirements for the issuance of new certificate of registration and what to do next. Print the notice and include it in the documents to be submitted to your concerned CDA Extension Office.

Length of Registration Process
All requirements for online application must be submitted to the CDA Extension Office within 15 days from the date of printed notice at the concerned CDA Extension or Field Office so that the Sr.CDS of CDA can immediately act on the cooperative’s application form. Attach the notice to the documents to be submitted. Incomplete requirements would further delay the issuance of the cooperative’s new registration number and Certificate of Registration.

How to check the status of your application

1. You can check the status of your application by visiting the log on page and entering your CIN. There are 2 possible statuses for your application: 1) in process and 2) CoR for printing. You can print the notice by clicking the “Click here to print notice” link. You can print the notice by clicking the Print button or save by clicking the Save button.

If you are absolutely sure that you have not submitted any application yet and you receive this message, there is strong possibility that your CIN has been used by other people. Proceed to or contact at once your concerned CDA Extension Office and report the matter.

2. If you get the message below, it means that your application has been approved by the Sr.CDS and Supv. CDS and that your cooperative has been issued a new registration number. The CoR will then be forwarded to the Regional Director of your concerned CDA Extension Office for signature. To claim your CoR, visit your concerned CDA Extension Office and pay the necessary fees.

For cooperative banks and electric cooperatives, your new CoR will be signed by the Chief of Registration and the Executive Director.

Your coop is classified as Non Operating message

The registration system accepts only cooperatives which are classified as “Operating”. If you believe that your cooperative’s status should be “Operating”, print the notice and visit your concerned CDA Extension Office immediately and present the necessary documents. Applicant must submit proof of operation to the satisfaction of the CDA Unit concerned which need prior written approval of the Regional Director/ Executive Director using attached form as RR Form No 2. The following documents may justify reclassification to operating cooperative: Audited Financial Statement as of December 2007 or 2008, Minutes of the General Assembly meeting, copy of Certificate of Good Standing for 2007 or 2008.

You can print the notice by clicking the Print button or save by clicking the Save button.

Your CIN was not found message

There are 2 possible reasons why this message would appear. First, you have incorrectly typed in your CIN or you are a spammer trying to guess a CIN to illegally enter the CDA Registration site. If you are a legitimate cooperative with a valid CIN issued by your concerned CDA Extension Office, then the error is most likely caused by an incorrect CIN entry. Visit your CDA Extension Office if you keep getting this message to verify that your cooperative was issued the correct CIN.

1st Mindanao Cooperative Peace Forum

Greetings!

We are pleased to announce the holding of the 1st Mindanao Cooperative Peace Forum on October 29, 2009 at the; Xavier University Gym, Corrales St., Cagayan de Oro City.

We, the more than five thousand active cooperatives in Mindanao with some one million five hundred thousand members, have joined hands with the Muslims, the Lumads and other indigenous Peoples, the Churches, the Ulamas, the Academe, the Rotarians and all peace-loving Mindanawons, to collectively advance what we believe in our inherent right to live in peace founded on social justice, equity and sustainable development.

We have witnessed hunger in a land described as the “Food Basket” of the nation and have experienced glaring poverty in an island oozing with rich resources. We decry that while Mindanao is rich in fisheries, agriculture, forests and mines, it is where 14 of the 20 people provinces are found and it’s poor are the poorest throughout the Philippines.

We realize that unless these contradictions are resolved, poverty will always breed violence; it will always remain as the best recruiter of terrorists. Unless the causes of dissent and conflict are dressed, military actions will just be palliatives and cannot provide the long term solution.

We are aware that the root cause of poverty in Mindanao is the social exclusion and powerless of the people to have access and control their resources. That as a fundamental step, the marginalized sectors must now harness their collective potentials, build their own coalition and make cooperative as the vehicle of their empowerment and development.

It is in this light that we are cordially inviting you to the aforesaid convergence. A subsidized Assessment / registration Fee of One Hundred Pesos ( Php 100.00 ) will be charge per participant to cover the cost of meals to include 2 snacks and lunch, kit and handouts materials.

For more information, inquiries, assistance and confirmation of attendance, please contact Ms. Gloria V. Barrido or Ms. Juhna Balundo of the CDA 10, tell. No. (088) 8576516 or email at cda_cdo@yahoo.com.ph or the CDA Office in your area.

Very sincerely,

Mindanao Public Consultation on the draft CDA-BIR Joint Revenue Regulation

The recently enacted Republic Act No. 9520, otherwise known as the Philippine Cooperative Code of 2008, lays down the tax and other fiscal incentives for cooperatives. However, under Article 144, par. 2 of the said law, the Cooperative Development Authority (CDA) and the Bureau of Internal Revenue (BIR) are mandated to jointly issue a revenue regulation to govern the implementation of these incentives and exemption.

It was noted that during RA 6938, the BIR enjoyed the unilateral power to issue Revenue Regulation 20-2001 regarding tax exemptions of cooperatives. Notably, under RA 9520, the CDA is mandated to participate in determining what provisions should be included in the new Revenue Regulation that would govern these incentives and exemptions. This time, Article 144, par. 2 of RA 9520 instructs the BIR and CDA to jointly issue the Revenue Regulation for this purpose.

In line with this, the CDA and BIR have already drafted the new Revenue Regulation and the same is new ready for public consultation.

For this reason, the Mindanao Public Consultation on the draft CDA-BIR Joint Revenue Regulation will be held on September 8, 2009 at the Crown Regency Residences, Cabaguio Avenue, Agdao, Davao City. A minimal registration fee of P180.00 per participant will be charged to cover the venue, snacks and materials during the activity.

PGMA inks cooperative code amendments into law

President Gloria Macapagal-Arroyo signed into law today the Philippine Cooperative Code of 2008, bringing to a successful end the campaign of cooperatives for a strengthened and more comprehensive law on the promotion and development of the country’s cooperatives

The signing ceremony was held at the Rizal Hall of MalacaƱang before lawmakers and representatives of the various cooperatives in the country.

President Arroyo signed the new Code, now know as Republic Act (RA) 9520, in the presence of its principal sponsors: Sen. Juan Miguel F. Zubiri, chairman of the chamber’s committee on cooperatives in the Senate, and his House counterpart, Cong. Ernesto C. Pablo.

The new law amends the 1990 Cooperative Code of the Philippines, otherwise known as RA No. 6938.

The new law gives cooperatives greater opportunities to serve their members, not only in terms of financial assistance, but also in undertaking more productive activities geared toward uplifting the lives of their members.

RA 9520 outlines in greater detail the requirements in professionalizing the management and operation of cooperatives, and provides for a monitoring and evaluation tool for the cooperatives to conduct self-assessments of its managerial, financial, and social objectives.

Under the new law, cooperatives are expected to play important roles in the country’s social justice and sustainable economic development programs.

Tapok-Tapok sa Koop ug Barkadahan sa Pasko 2008

FUN AND PRIZES DURING COOP GATHERING
By Henrylito D. Tacio

THEY came all the way from different provinces, they shared their talents and skills, and they went home with prizes and so much fun.

Such was what transpired during the "Tapok-tapok sa Kooperatiba and Barkadahan sa Pasko 2008" held at The Venue recently. More than 1,000 cooperative members from the three provinces of Davao (del Sur, Oriental and Del Norte) and Compostela Valley attended the gathering.

"Our prime intention is to build camaraderie and close relationship among all cooperators from Boston, Davao Oriental to Sarangani, Davao del Sur, regardless of rank and status in their cooperatives," explained Rosalinda E. Villaseca, the administrator of Cooperative Development Authority (CDA) and the concurrent regional director and overseer of the agency in Regions 11 and 12.

In addition, the gathering was also a way of strengthening the Cooperative Development Council in Region 11 as "a premier support mechanism for the cooperative movement in policy advocacy," Villaseca said.

The unique activity -- or "grandiose affair" to quote the words of Atty. Fulgencio A. Vigare, Jr., the CDA administrator from the Department of Finance – started five years ago. "This annual gathering is the first of its kind in the Philippines," Villaseca said.

Because of its success, it has caught the attention of other cooperative leaders from other regions in the Philippines. During the recent affair, cooperative representatives from Luzon and Visayas came to witness how the program was being conducted.

Taking advantage of the festive mood of December, the gathering adopted the strategies of fun and prizes. Randy Albores, who won a national prize for composing the country's cooperative theme song, hosted the show which started at 9:00 in the morning until 4:00 in the afternoon.

Among the competitions that were held were Coop Idol '08 (won by representative from Davao del Sur), Sayawitan Contest (Davao del Norte emerged the winner), Gary V. Sing and Dance Alike Contest (won by a lone contestant from Davao del Sur), Raprapan sa Coop Contest (again won by a lone contestant from Davao del Sur), and Coop Can Dance Contest (lucky winners were from Davao del Sur).

Winning the most number of delegates was Davao del Norte, with 330 registered participants. Davao City closely trailed with 259, then Davao del Sur (189), Compostela Valley (185) and Davao Oriental (69). Total registered participants (excluding guests and visitors): 1,032.

During the opening program, Davao City Mayor Rodrigo Duterte welcomed the participants with a message, which was read by his representative, Engr. Jaime G. Adalin.

"Cooperatives have been credited for their major contribution in the development of communities," Mayor Duterte said in his message. "This is proven by the diverse business activities which have mushroomed in the city and neighboring areas and greater people participation in socio-economic activities."

Mayor Duterte said he is fully supporting programs "designed to strengthen the cooperativism in this part of the country." He urged cooperative leader "to create a vision, and focus on goals more than the details of operations."

"Good governance," he added, "must also be emphasized to safeguard the trust and confidence of the members and other stakeholders." -- ###

Mindanao Cooperative Cancer Society (MCCS)

Hope against breast cancer
By Henrylito D. Tacio

AMONG Asian countries, the Philippines has the highest prevalence of breast cancer. Since the 1980s, breast cancer ranks number one among the top leading cancers afflicting women and ranks second to lung cancer between both sexes.

Recent data show that 6,360 breast cancer patients die each year in the country, making it the single leading cause of death among Filipino women.
A diagnosis of breast cancer is indeed a harrowing experience, especially when one considers the statistics that go with the disease. "I first noticed the lump in my breast in the last week of December 2006," recalls Rosalinda E. Villaseca, the founding president of the Mindanao Cooperative Cancer Society (MCCS). "Initially, I felt a pain in my left armpit."

She reckoned the pain must be due to the stressful driving she did from Panabo to Davao and then back to Panabo. But after reading some magazine articles, she surmised that the lump must be something else. So, she went to a doctor and was diagnosed of having a breast cancer.

Villaseca was totally terrified upon hearing the diagnoses, but after realizing that everyone has to die anyway, "I accepted it fully as His will," she said.

Considering that she was already 62 years old at that time, "I am ready to return to Him as our life in this world is just borrowed."

Because of the readings she did on the effects of chemotherapy, she objected initially of undergoing such treatment. It took a month of convincing from her doctor and family before she subjected herself to chemotherapy.

She survived the ordeal.

"Many were surprised because prior to the discovery of the disease, I looked good, very healthy in appearance and no semblance of sickness, whatsoever," she said. "They were even more surprised when they saw me after the chemotherapy treatment. My appearance seemed to improve better and there were no traces that I have been a breast cancer patient."

Life has been good to her. When she was diagnosed of having a breast cancer on January 4, 2007, that was also the time she was notified on her appointment as administrator of Cooperative Development Authority (CDA) from MalacaƱang. "God is really good," she said.

She is not only the CDA administrator but she's also the concurrent regional director and overseer of the agency's offices in Davao Region and Central Mindanao.

Actually, her academic background was agriculture having graduated with such profession from Central Mindanao University in Musuan, Bukidnon in 1966. However, she finished a master's degree in public administration from Notre Dame University in Kidapawan City in 2000.

Villaseca's work with CDA spearheaded her to put up MCCS. "I have spent half a million pesos (more or less) in the chemotherapy treatment alone, not to mention the cost of the mastectomy operation and hospitalization and other laboratory tests and incidental expenses," she said.

She couldn't imagine how poor women with breast cancer could cope up with the high cost of treatment. "As a government employee with a high pay, I still suffered from high cost of medication and hospitalization especially the 6 sessions of chemotherapy treatment," she said.

This was one of the main reasons why she launched MCCS during the Fourth Mindanao Cooperative Summit held in 2008 in Cagayan de Oro City. As a non-stock, non-profit organization, MCCS is not a business and social enterprise.

"Business is not the consideration of MCCS," she clarified. "It is solely for service through education, promotion and the coming up of basic and vital information material to help the public be aware of the do's and don'ts of the disease."

In real sense, MCCS does not differ so much from the existing cancer societies in the country today. "We only wanted to focus our assistance to the poor members of the cooperatives in Mindanao," she said.

The MCCS is the first cooperative cancer society established in Mindanao. Just like other cancer societies in Luzon and Visayas, it intends to complement rather than compete.

"The overarching intention of MCCS is to bombard the less-privileged members of the cooperatives with the correct and appropriate information about this disease down to the grassroots level," she said.

Today, MCCS has 23 individual members aside from 10 other organizations. "We do not limit to members of cooperatives or cooperative organizations only," she said. "We welcome anybody who is with us in this crusade."

Being a new society, it doesn't have any capacity yet to help members who are suffering from breast cancer financially. "The only thing we can provide or offer are words of encouragement, hope, prayers for them to have strong faith in the Lord," she said.

Based from her experience, she pointed out, "Positive disposition of a person is one vital attribute for fast recovery of a person."

Villaseca is a portrait of a breast cancer patient. "My strong faith in the Lord -- of Him being the Great and Almighty God and source of all lives -- is the secret of the full restoration of my health in body and mind," she admitted.

Her final thoughts: "Life is short, but not too short for us to show gratitude to God -- in grateful appreciation of what He has done to me, to my being, and to my career and health. I hope it is not too late for me to repay Him by doing these God-given tasks of promoting the principles of prevention rather than cure."

In loving memory of the late Nilo Quilab

We unite ourselves in prayer for the salvation of the soul of CDA Davao driver, the late Nilo Quilab. He returned to our Creator last September 4, 2008 after four days of treatment at Davao Medical Center, Bajada Davao City. Doctors said that aneurism was the cause of Nilo's death. An aneurism is a weak spot in the wall of an artery or a vein that dilates or balloons out, forming a blood-filled sack or pouch. Aneurisms can occur almost anywhere in the body and are found in all age groups, although they occur primarily in the elderly. The foremost cause of aneurisms is atherosclerosis, or fatty deposits in the arteries. If an aneurism bursts, a massive amount of blood is released, which results in an almost instantaneous drop in blood pressure and can cause death. Surgery can be a successful treatment for unruptured aneurisms.

May the souls of the faithful departed rest in peace .
